## Notice Under the Americans with Disabilities Act
In accordance with the requirements of the Americans with Disabilities Act of 1990 (ADA), Northwest Optical will not discriminate against qualified individuals with disabilities on the basis of disability in its services, programs, activities, or employment practices.
The Northwest Optical website provides a third-party accessibility service designed to assist visually impaired individuals and others who may require accessibility accommodations when viewing the website on a desktop computer, tablet, or mobile device. Some portions of the website may not be fully compatible with this service. Files and content such as photographs, graphics, documents, embedded content, and information provided by third parties may not adjust or respond in the same manner as other portions of the website when accessibility software is being used.
Northwest Optical does not discriminate on the basis of disability in its hiring or employment practices. Northwest Optical will not ask a job applicant about the existence, nature, or severity of a disability. Applicants may be asked about their ability to perform specific job functions. Medical examinations or inquiries may be made after a conditional offer of employment when appropriate and when such requirements are applied consistently to applicants for the applicable position.
Northwest Optical will make reasonable accommodations for the known physical or mental limitations of a qualified applicant or employee with a disability upon request unless the accommodation would cause an undue hardship on the normal operation of Northwest Optical. To the extent that selection criteria used for employment decisions have the effect of disqualifying an individual because of a disability, those criteria will be job-related and consistent with business necessity.
Northwest Optical will make reasonable modifications to policies and procedures when necessary to provide people with disabilities an equal opportunity to access and enjoy its services and activities. Individuals with service animals are welcome at Northwest Optical locations in accordance with applicable laws and regulations. For off-site functions or events held at third-party locations, the accessibility policies and procedures of the applicable venue may also apply. Individuals are encouraged to contact the venue in advance or contact Northwest Optical directly with questions or concerns regarding accessibility.
A person who requires an accommodation, auxiliary aid, or other service to participate in a Northwest Optical service, program, appointment, or activity should contact Northwest Optical as far in advance as reasonably possible. When an accommodation relates to a scheduled event or activity, Northwest Optical requests that individuals provide notice at least 48 hours in advance whenever possible.
Complaints or concerns that a Northwest Optical service, program, activity, facility, or website is not accessible to persons with disabilities should be directed to Northwest Optical for review. Complaints regarding disability-based discrimination involving applicants for employment or Northwest Optical employees should also be directed to Northwest Optical for appropriate review.
Northwest Optical will not impose a surcharge on an individual with a disability, or on a group of individuals with disabilities, to cover the cost of providing reasonable accommodations, auxiliary aids or services, or reasonable modifications to policies and procedures when such accommodations are required.
